Most people in organisations which we surveyed told us that problem at the top was it did not follow the chain of command. Strong logic. But look at what happens..if the 'Chain of Command' is only a non-value adding or superflous value adding delivery chain which delivers work done at the bottom of the pyramid to the top of the pryamid...no doubt the temptation of the Top is to by-pass the chain for that can only add to the time and cost of delivery. Bite the bullet...cut the flab. Add a layer and level only if it adds to value. Superfluous supervision also adds to the chaos as each level has to justify its existence. No doubt we are in an age of flat structures.
